Multiple Equipment Form
This form is used if additional space for equipment information is needed on the Lease Agreement.

Certified Signer Form
This form is used to certify that the individual named on the lease is legally authorized to sign documents on behalf of the business entity on the Lease Agreement.

Method of Payment Form (ACH Authorization Form) - Commercial / Consumer
This form is used to identify how the lessee chooses to pay the required schedule of payments, with the following options:

  • Option A: EFT Direct Payment (requires copy of voided check)
  • Option B: Credit Card – Direct charge to a credit card (VISA, MasterCard, AMEX, or Discover)

$1.00 Buyout Purchase Option
This form is used only with certain Credit Score A transactions in which the lessee has a $1.00 Purchase Option at the end of the lease term.

Conditional Purchase Order Form*
This form serves as verification that the equipment listed has been approved for payment based on previously issued credit terms and conditions.  *This form is only valid when received and signed by an authorized TimePayment Representative.*

Commercial Lease Agreement with Bill of Sale **
This is a lease agreement that includes a Bill of Sale in lieu of an invoice.  **Please note this form is only used for Bankcard equipment leases.**

Blanket Resale Certificate Form
This form is provided by TimePayment to our Vendors as a Tax Exempt Certificate for equipment sales.

TimePayment W-9 Form
This form is provided by TimePayment to our Vendors for tax purposes.
